As stated in Memo 2012/01MP-Signage, this is the initial contact for you to provide us with information regarding the signage at your centre. As we are replacing signage at all our centres, we need your full cooperation regarding the specifications of your signage. Please email this completed document to marius@mastermaths.co.za, or fax it to 021 851 5300. If you have any problems filling out this form, please call Marius at head office for assistance. If you have more than one centre you will need to fill out a separate form for each centre.
As stated in the memo, if we do not receive feedback from you within two weeks you will be moved to the bottom of the list.
Please make sure you provide the correct sizes, phone numbers and any other information. You will be sent a proof once we have designed your signage, after which we will then send the design to the printers. If the printed sign is incorrect due to wrong information you supplied (after you have checked it), you will not be supplied with new signage material. You will then have to pay for the replacement artwork since keeping out-dated signage is not an option. Also, be sure that the phone numbers which you include will not change in the near future. If your number does change you will have to replace the entire signage at your own cost.
You will need to supply us with a front view digital photo of your centre and all current signage. If you do not include photos of your signage with this form we will not be able to process your request.
Please note: The signage artworks need to be 'installed' within a month of receiving it. You need to send us a picture of all the signage items once they are installed, or you will be billed for the cost.
With this form do not include:
* Any advertising requests other than signage
* Window signage
* Agent boards
* Street pole boards/ads printed on paper or cardboard
*Important note for centre signs next to a busy road: Signs that are at an angle (see example) are far more visible than those on a flat surface. Angled designs are also more effective if you have Master Science and we need to combine your signage. If you are a new franchise or an existing centre and want to upgrade your signage, it is advisable to install angled signage. For these designs you will be supplied with two signage stickers.
